We appreciate you taking a few minutes to complete your feedback survey after your admission.
Your feedback is anonymous, unless you choose to disclose your details when submitting your survey and wish to be contacted.

What is the Cemplicity Patient Experience Programme about?
The Cemplicity Patient Experience Programme is a survey that aims to measure patients’ experiences of various aspects of care in our hospital. This survey may take around 5 minutes to complete.
What is the purpose of the survey?
At Healthwoods Endoscopy Centre, we are committed to delivering excellent service to our patients. Your feedback is a valuable way for us to understand where and how we can improve. As a result of the Patient Experience Program we hope to see improvements in our quality of services and the experiences and outcomes of our patients.
How do I participate?
Around 10-15 days post discharge you will be invited, via email, to fill in a short questionnaire. If you wish to help us and would like to participate, fill in the questionnaire with your answers using the link provided in your email invitation. You may receive an email reminder the following week to complete the survey. The link to complete the survey will be open for 21 days.
Do I have to take part in the survey?
Your help would be greatly appreciated, but it is not compulsory. If you do not wish to take part, do not complete the questionnaire.
Is the survey anonymous?
Your response will remain anonymous unless you choose to provide your contact details to be contacted. We will not see any personally identifiable data with your response.
What if I change my mind?
If you change your mind part-way through the survey, and no longer want to complete it, simply close the web browser. If you do decide to complete the survey, you can follow the same link and it will take you to what you last completed in the survey. Your response will only be submitted once you select the ‘submit’ button at the end.
